Antenatal Cares in Five Medical Institutions in Seoul, Korea


Abstract
This paper is a preliminary report on the clinical aspects of the antenatal cares at five medical institutions in Seoul, Korea. The purpose of the study was designed primarily for the clarification of the possible association with the outcome of pregnancy subsquent to the previous induced abortion.
In order to implement the study, practically all pregnant women visiting antenatal clinics at four university hospitals and National Medical Center located in Seoul, Korea were interviewed. Samples include pregnant women with experience of induced abortion in the previous pregnancy, primigravida, multiparous women and pregnant women with previous spontaneous abortion. It is worthy to note that the study has been designed to collect data by a prospective study and to carry out jointly with institutions in England, Denmark, Poland and Sweden as well as the internal collaboration within Seoul. As a prospective study this is a unique expansive venture in the field of antenatal care activity in Korea ever implemented. The study will be continued for sometime, however data accumulated so far is considered to be enough to elucidate some fact on antenatal cares in Seoul. Therefore, this paper is presented as a preliminary report.
The result observed as of the end of May, 1975 are summarized as follows;
1. The period covering the present report began from February 2, to May 27, 1975 and total number of samples interviewed at five institutions are 1, 272 women.
2. The proportion of women who visited antenatal clinics before 12th week of gestation is in 15.3%, during 13th and 24th week 25.6%, from 25th to 36th week 38. 2%, 37th to 40th week 17.2% and beyond 41st week 3. 8% respectively. Clients seeking antenatal care at governmental institutions tend to come in late pregnancy in comparison with the private institutions.
3., More than half of the samples (54. 1%) was priorly, diagnosed as pregnancy at other private clinics than five participating institutions. About 32% had their childbirths at other private clinics shifting from five institutions where interviews and registration had been made, .3.6% had home deliveries. Thus about 65% of women under the study had deliveries at the same institutions where interviews made.
4. Regarding age of the expectant mothers, women in 20¢¥s comprise 78% of the total, those in
30¢¥s 21. 1%. The proportion of women in 30¢¥s is decreased by 13% in comparison with the age distribution of antenatal population one decade ago, which is considered to be due to the earlier limitation of family size at present.
5. Of all cases interviewed 45.3% is represented by women attaind college levels, whereas. among the general population in Seoul the comparable figure remainds at 5%.
6. Proportion of nuclear family comprises 70% of the total, no significant difference with the general population in Seoul.
7. Primigravida constitutes 32.5% of the all women interviewed. The remainder have experienced 2.32 pregnancies each on average, exclusive of the current pregnancy.
8. Of all women interviewed, 32.6% have ever experienced induced abortion in their past. Of 858 women who has prior experience of pregnancy 48.4% has had induced abortion. Induced abortion comprises 36.4% of all previous pregnancies experienced by the samples.
9. Of total samples, 1,272 women spontaneous abortion was reported in 17.5%. which corresponds 26.0% of women ever experienced pregnancies in the past, and 11.2% of total pregnancies experienced in the past. 2.8% of women have experienced stillbirths, corresponding 1.8% of all pregnancies.
10. Of 858 women, their last pregnancies were terminated as live birth in 44.2%. The remainder, 55.8% was pregnancy wastage; specifically stillbirth in 2.6%, induced abortion in 36.8%, and spontaneous abortion in 16.4%.
